Pricing and Who We Work With

At Hand in Hand Support Coordination, all Support Coordination fees are included in your NDIS Plan under the ‘Capacity Building’ support category. A detailed price guide is available on the NDIS Commission’s website at www.ndis.gov.au or through this link.


If this category is not included in your plan, you can request a review by the NDIA. We welcome TAC (Transport Accident Commission) and WorkSafe referrals and support various funding arrangements across Melbourne and Victoria.

  • How do I apply for the NDIS?

    To apply for the NDIS, you need to meet specific eligibility criteria. You must:

    •  Have a permanent disability that significantly impacts your daily activities.
    • Be an Australian citizen, permanent resident, or hold a Special Category visa.
    • Be under 65 years of age when applying.

    If you do not currently receive support but wish to apply, contact the NDIS at 1800 800 110 and request an Access Request Form.

  • What evidence do I need to provide when applying?

    When applying for the NDIS, you must provide evidence of your disability that:

    • Confirms your primary disability type and the date it was diagnosed.
    • Describes how the disability affects various areas of your life, such as mobility, communication, self-care, etc.
    • Details treatment history and expected future outcomes.
    • Is relatively recent (within the past 6-12 months).
    • Is completed by a relevant treating health professional, such as a GP, Paediatrician, Occupational Therapist, Neurologist, or Psychiatrist.

  • Who can provide evidence for my NDIS application?

    Evidence should ideally be provided by a treating health professional who is familiar with your condition and has seen you for at least six months. This could include:

    • General Practitioners (GPs)
    • Occupational Therapists
    • Psychologists or Psychiatrists
    • Neurologists
    • Speech Pathologists

    These professionals should know the required assessments or reports to provide, which can be found on the NDIA's relevant assessment list.

  • Where can I find more information on eligibility and conditions?

    The NDIA provides lists to help determine eligibility. You can refer to:

    • List A: Conditions likely to meet the disability requirements.
    • List B: Permanent conditions with variable functional capacity.
    • List C: Defined programs.
    • List D: Permanent impairment/early intervention for children under 7.

    These lists are available on the NDIA website and can provide detailed guidance on what conditions meet specific requirements.

  • What funding arrangements do you accept, and how are Support Coordination fees managed?

     At Hand in Hand Support Coordination, we accept various funding arrangements, including NDIS-NDIA-managed, self-managed, plan-managed, and self-funded clients. All Support Coordination fees are included in your NDIS Plan under the 'Capacity Building' support category. If this category is not present in your plan, you can request a review by the NDIA. We also welcome referrals from the TAC (Transport Accident Commission) and WorkSafe, and our team can connect you with a Client Independence Skills Services (CISS) officer for further assistance.
